I know for the longer tennis matches (2.5 hours +), they usually have a bathroom break allowed for the players if they request it. However they normally only happen at the end of a set in which the break is long enough for them to do their business.

However it's usually a very big deal if it happens in the middle of a game or even a set because it disrupts the flow of the play so much.
