if it is the team who advised the athlete to take the medication, doesn't a large part of the responsibility fall upon the team?
For the F1 example, not only the driver is penalised but the car is penalised. For an athlete, sometimes we see the coach being penalised. If the athlete has already asked if the drug is ok (actually many people will have a lot of trust and not ask), and/or the team hasn't explained the implications properly, then medical legal law will side with the plaintiff.
I read that LCW has had stem cell treatment - the last paper I read on it was from the international Olympic committee reviewing the evidence. There is very sparse evidence of being of benefit....and what of the harm?

1) heads will roll from medical team. If it is due to sickness/ injury that drug was prescribed and they disnt fill up that TUE form
2) BAM will have their punishment already with loss of sponsors if LCW has to retire now.

Whether BAM president will resign if LCW sample B test positive, depends on his conscience and the political clout he holds, literally. If he has powerful connections, then smaller minions will get the chop.

If an athlete has something like a golfer's or tennis elbow tendinitis and gets steroid injection into that area for faster recovery, is that a case for a possible TUE application?
Yes bro regardless of how it is administrated since such drugs is prohibited under the 2014 WADA list be it through oral, IV, IM or rectal route ( S9 / 2014 WADA prohibited list)

Right, I see corticosteroids (the class of steroids that dexamethasone belongs to, not anabolic steroids) are only prohibited in-competition. So they're allowed for out of competition treatment of injuries and such.
Then someone in his medical advisory committee must have miscalculated, thinking that the dexamethasone used to treat his thigh injury in around early July would be clear of detectable levels by WC in late Aug.
